Listed below are the necessary steps and forms needed to complete for enrollment at Maranatha Christian Preschool. Please bring all completed forms to the preschool office.

Preschool Admission Process

STEP 1 ~ Please download, read and sign all forms. It is very important to read each of the listed forms. Prior to attending school the State Law requires each to have a physical exam, and up- to-date immunizations.

STEP 2 ~ Bring all forms to the preschool office. The enrollment packet must be completed before the first day of attendance.

Application for Enrollment 10-11 - REQUIRED -Print, sign and return to preschool office
Physician's Report - REQUIRED -Print, sign and return to preschool office
Parent's Report - Health History - REQUIRED -Print, sign and return to preschool office
Identification and Emergency Information - REQUIRED -Print, sign and return to preschool office
Medical Consent - REQUIRED -Print, sign and return to preschool office
Parent's Rights - REQUIRED -Print, sign and return to preschool office
Personal Rights - Personal Rights - REQUIRED -Print, sign and return to preschool office
Admissions Agreement - REQUIRED -Print, sign and return to preschool office
Parent Handbook/Permission to Print Photos - REQUIRED -Print, sign and return to preschool office
Earthquake/Disaster Form - REQUIRED – Print, sign, complete emergency kit and return to preschool office
California Immunization Card REQUIRED –Submit to preschool office for verification
Parent Immunization Guide - READ ONLY
Immunization Clinic Schedule - READ ONLY
State Background Check Process - READ ONLY
